Go to https://www.surveymonkey.com/ and sign in with your credentials. Click on the square icon found at the top right-hand side of the page.
This will load a dropdown menu, click on Integrations & Plug-Ins
This will take you to another page. Click on My Apps, found in the menu bar across to top of the page.
Click on Add a New App. 
Fill in the required details and select private app. Once completed click on Create App.
Make a note of the client ID and secret and then click, Deploy.
